Education & Training: All Hypnotists at ROC Hypnosis / JAX Hypnosis are Certified Consulting Hypnotists (C.H.), who have met all criteria and training to meet the standards set by certifying organizations. All Certified Consulting Hypnotists at ROC Hypnosis / JAX Hypnosis are insured and bound by a strict CODE OF ETHICS. All Certified Consulting Hypnotists at ROC Hypnosis / JAX Hypnosis must maintain annual continuing education training in our field to keep within the highest level of professional service.
Notice: AS THE STATES OF NEW YORK AND FLORIDA HAVE NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by state or Federal governments. We are not physicians, nor licensed health care providers and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. A client has the right to be free of physical, verbal or sexual abuse. A client has the right to know the expected duration of sessions, and may assert any right without retaliation.
Fees: Fees for services vary depending upon clients' concerns being addressed, length of sessions required to deliver such services, methods employed, and number of sessions required to address the needs of said client. Prior to services being rendered, fees for such services will be noted and agreed upon. All fees are due prior to services being rendered. Payment must be made in the form of credit/debit card, HSA or FSA benefit cards, or Bank transfers. There are no refunds on services rendered. Refund policies will be disclosed in writing prior to agreement of services. Fees compensate hypnotist for his/her time agreement & commitment for services to client, along with time required to create the clients program before sessions begin, regardless of results. Success of clients' sessions are based on said clients' willingness to follow designed programs, to include: keeping scheduled appointments, listening to assigned audios, and application of assignments given. Unless appointments are changed or canceled within at least 12 hours prior to scheduled appointment time, you will either forfeit said session or may opt to pay a $50 rescheduling fee, due at time of rescheduling.
Confidentiality: While we are not bound by HIPAA, we will not release any information to anyone without written authorization from you, except as provided for by law. All documented records are property of ROC Hypnosis / JAX Hypnosis, and will only be released via a court order.
Insurance: We do not work directly with insurance providers. However, we will provide you with a receipt, so you may submit to your provider, increasing your chances of reimbursement, if desired. In most cases, Flexible Spending Accounts (FSAs) and Health Savings Accounts (HSAs) typically cover our services, but coverage is not guaranteed.
